New fibreoptic duodenoscopes allow an experienced endoscopist to visualize and cannulate the papilla of Vater in conscious patients with at least an 80% chance of achieving retrograde cholangiopancreatography-a technique with obvious diagnostic potential. The historical aspects have already been ade-quately covered.'-4 This review concerns the rapidly increasing world experience, with particularregard to technical problems, clinical relevance, and safety.
