Under normal conditions, a stimulus to change the accommodation of the eye is ac companied by a change in the stimulus of convergence, which will be manifest as a change in the phoria of the eyes. The magni tude of the change of convergence (phoria) in prism diopters caused by a given change in the accommodation (diopters) is called the" accommodative convergence: accommo dation ratio"(AC: A) and is usually ex pressed in prism diopters per diopter. This ratio can be found in several ways. The easiest way is to determine the phoria at a given fixation distance and then add a plus or minus lens before the fixing eye and again measure the phoria. The amount of change in the phoria (convergence) brought about per diopter change in accommodation gives the AC: A ratio. This ratio may be useful clinically in problem cases in which sympto matic heterophorias must be dealt with, especially in early presbyopia, in other nearvision problems, and also when some insight into the magnitude of the accommodative innervation is desired. 1
