Summary Biopsies of cirrhotic livers studied with the electron microscope showed the presence of modified fibroblasts (‘myofibroblasts’) in the fibrous tissue. These cells share the morphological characteristics of smooth muscle cells and fibroblasts. In pharmacological studies other workers have shown that strips of granulation tissue from healing wounds possessing such cells contract under the influence of various vaso‐active agents in the manner of smooth muscle. It is suggested that the presence of such contractile cells in the scars in cirrhotic livers may be an important contributing factor in the pathogenesis of portal hypertension and the progression of the cirrhotic process.
