A leukemoid reaction to multiple myeloma enters into the differential diagnosis of chronic neutrophilic leukemia since in both conditions toxic granulation and Döhle bodies can coexist with neutrophilia. Occasional patients have been reported with coexistence of the two conditions. The patient reported here illustrates a more complex situation.
