A 77-year-old Chinese woman presented with a 10-year history of persistent painful perianal nodules. She reported intermittent swelling of these nodules, associated with a purulent discharge, which was partially controlled with courses of oral antibiotics. There was no associated fever, lower gastrointestinal symptoms or other systemic problems. The patient had hypertension and hyperlipidaemia. There was no relevant family history. On physical examination, multiple erythematous indurated nodules were seen in the perianal region, with a background of surrounding hyperpigmentation. Some of the nodules were exuding a purulent discharge. Laboratory investigations identified a mild leucocytosis, and the erythrocyte sedimentation rate was significantly raised at 80 mm ⁄ h (normal range 0–20 mm ⁄ h). A punch biopsy was taken from one of the nodules.
