DISCUSSIONThe stria vascularis, which produces endolymph and maintains the endocochlear potential via active transport, is essential for the mechanoelectrical transduction of sound (1). Atrophy of the stria vascularis, which is thought to be genetically influenced, classically results in a progressive, bilateral symmetric sensorineural loss in a flat configuration (3).In this case, a female patient presented to the otology clinic for surgical management of right-sided chronic otitis media. Over the course of 12 years, serial audiometric examination was performed, revealing symmetric, progressively worsening bone conduction thresholds consistent with atrophy of the stria vascularis. This was confirmed on postmortem examination. Although not uncommon, stria vascularis hearing loss likely represents an underrecognized cause of presbycusis.
