The pathologic features of BPD have been delineated, but there are few morphometric studies in which the lungs of infants with BPD have been compared with those in infants without neonatal lung injury (5-9). Decreased total alveolar number has been reported in the lungs of ventilator-dependent, premature infants with or without hyaline membrane disease (10). In a separate study, a marked reduction in total alveolar number and internal surface area was reported in a 34-month-old child with long-standing, healed BPD (8). In that patient the small airways were similar in size and density to those of three children who died without lung disease at 26, 34, and 41months of age, respectively.In this autopsy study wequantitatively examined the airwaysand pulmonary parenchyma of eight children with reparative and chronic BPD and six children without chronic lung disease. The results of our study indicate that the lungs of patients with BPD havegreatly decreased alveolar number and internal surface area, large alveolar diameters, and an abnormal distribution and structure of elastin fibers. Wealso observed remodeling of the large and small airways, with regional variability of remodeling in the central bronchi. Our results indicate that
