Comments on an article by B. Kerková et al.(see record 2024-26258-030). Kerková et al. recently reported in this Journal that CHT significantly moderates the relationship between increased inflammation and cognitive outcome, with some specificity across cognitive domains. We agree, and would argue that genetics, epigenetic factors (EP), substance abuse and other social determinants of health may also be at play in concert with, or in a cascade of multi-hit etiological processes. Additionally, inflammation may be one of the mechanisms that interacts with or mediates other processes such as alterations in glutamatergic function, hypothalamic-pituitary adrenal axis and oxidative stress in the pathophysiological cascade that leads to psychopathology which includes not only cognitive impairments but also psychosis and depressive symptoms. Other social determinants of causation of schizophrenia and related psychotic disorders include poverty, ethnic discrimination, low ethnic density, migration, social fragmentation and homelessness. These observations are in general consistent with the multiple hit models of SZ and other mental illnesses, which highlight the possibility of intervention at several timepoints. Genetics findings suggest largely overlapping risk across mental diseases and functional outcomes, and an important role of CHT as a first hit in the trajectory toward psychosis.(PsycInfo Database Record (c) 2024 APA, all rights reserved)
