A 29 year old gravida 1 woman underwent ultrasono• graphic evaluation at 26 weeks' gestation. Examination of the fetal genitalia revealed a normal scrotum and testes with no phallus present (Fig. 1). The kidneys appeared normal; however, no bladder was visualized. An omphalocele was present with a low insertion of the umbilical cord (Fig. 2). Amniotic fluid volume was normal. The differential diagnosis included bladder exstrophy with aphallia versus a cloacal variant, and plans were made to effect delivery at a tertiary care center. The 2400 g infant was born by spontaneous vaginal delivery at 35 weeks' gestation with Apgar scores of 9 and 9.
