The avulsed skin should be saved for recon-struction, when viable, by immediate split-thickness (or, preferably, full-thickness) grafting or microsurgical techniques to reduce sequelae." Partial-thickness meshed grafts offer poor results, and the other emergency procedures are at risk for complications when ex-tensive muscle injuries are present. We propose a new approach when an ex-tremity (proximally to the wrist or ankle) is severely injured and degloved: emergency preparation and defatting of the avulsed skin and cryopreservation; open management of the wound in the extremity and second-look debridement, if required; and finally, when the recipient bed is clean, granulation tissue has circumferentially wrapped around the extrem-ity, and the patient is resuscitated, the previously prepared skin is applied as a single-piece, full-thickness graft.
