A 27-year-old woman, previously in excellent health, presented to Vanderbilt University Hospital because of accelerated hypertension, proteinuria, and azotaemia. The patient had been seen by her local physician 7 months earlier for oedema, when she was noted to have a positive test for protein in her urine, and also found to be in the 3rd month of pregnancy. Her serum creatinine at the time was 0.7 mg/dl. Throughout the course of her pregnancy, however, her renal function worsened (creatinine reaching 1.7 mg/dl) and urinary protein excretion was elevated at 7.0 g/24 h. Labour was induced, and she delivered a healthy baby. Upon admission to the hospital she was noted to be hypertensive with a blood pressure of 190/128 mmHg. The physical examination was also remarkable for a prominent apical impulse, a fourth heart sound, and 2+ bilateral pitting oedema of the feet, ankles, and legs up to the knees. Laboratory evaluation was remarkable for a creatinine of 3.1 mg/dl, albumin of 2.1 g/dl, hypercholesterolaemia, and negative serological work-up, including tests for lupus, complement, cryglobulins, and antineutrophil cytoplasmic antibodies. Urinalysis revealed microscopic haematuria and numerous coarse granular casts. Following normalisation of blood pressure with antihypertensive medications, a renal biopsy was performed.
