We read the paper written by Tominaga et al.[1] entitled “Effect of the duration of previous osteoporosis treatment on the effect of romosozumab treatment” in the June issue of Osteoporosis International with great interest. Their conclusion indicated that treatment with oral bisphosphonate (BP) or denosumab (DMAb) for> 1 year attenuated the effect of romosozumab (ROMO) evaluated by the change of bone mineral density (BMD) in lumbar spine (LS), but not in total hip (TH). This finding was impactful, particularly in the treatment initiation and duration of BP and DMAb. We reported on the treatment response of ROMO [2, 3], and analyzed data similarly regarding the effects of previous osteoporosis treatment duration. In their report [1], mean age was 77.2 years, and BMD T-score was LS=− 2.4, TH=− 2.4. However, mean age was 74.5 years, and BMD T-score was LS=− 3.1, TH=− 2.6 in our group (Supplementary Table 1)[2]. Duration of previous BP orDMAb treatment≤ 12 months was similar. Patients were divided into five groups: previous treatment naïve (n= 55) group, BP≤ 12 months group (n= 10), BP> 12 months group (n= 27), DMAb≤ 12 months group (n= 16), and DMAb> 12 months group (n= 29). Both BP groups, regardless of previous BP duration, showed a similar transient increase in total N-terminal type I procollagen propeptide (PINP), and a decrease in isoform 5b of tartrate-resistant acid phosphatase (TRACP-5b)(Fig. 1a and b), although their “dual effect” was smaller than the naïve group. However, there was a rapid and higher increase in both PINP and TRACP-5b in the DMAb> 12 months group compared to the DMAb≤ 12 months group. The naïve group showed a significantly higher BMD increase in LS compared to all other groups (Fig. 1 c and d) after 12 months of ROMO treatment. The naïve group tended to show a higher increase compared to BP groups and the DMAb≤ 12 months group concerning TH BMD, but the increase was significantly higher in comparison to DMAb> 12 months group.
