Since IgA is the major immunoglobulin component of certain local secretions (80, 81),(ie those obtained from organs having direct access to the external environment) it was anticipated that the study of IgA deficient states would lead to an understanding of the local immune system. However, controversy exists as to the clinical significance of selective IgA deficiency. Some reports describe this deficiency in clinically normal individuals (39, 67) while other larger series describe selective IgA deficiency in association with significant disease (16, 17, 47, 89). It is the purpose of this review to present an extensive analysis of the spectrum of selective IgA deficiency states based upon a review of over 150 cases from the literature and our experience with 30 patients. Selective IgA deficiency is defined as follows:1–less than 0.05 mg/ml serum IgA. 2–no deficiency of other immunoglobulins, eg IgG, IgM, IgD, or IgE. 3–normal cellular immunity. 4–normal humoral antibody production. Correlation of the data obtained in our studies with a review of patients described in the literature permits further elucidation of the interrelationships of selective IgA deficiency and disease.
