The possibility that sleep-induced arrhythmias may be a factor in the Sudden Infant Death Syndrome (SIDS) is based on two considerations. The first is that death appears to occur during the nocturnal period, when the infants are presumed to be asleep.'The second is that there is growing evidence that some of the SIDS events may be cardiac in rigi in.^-^ However, since there have been no electrocardiographic records of the infant deaths, discussion of this subject must necessarily be inferential. Several tacks will be pursued in an attempt to provide insights into the possible mechanisms whereby the sleep/wake cycle can conduce to arrhythmic death in infants. The first will be to discuss the effects of sleep states on cardiac arrhythmogenesis in adults. The second will be to review experimental studies of the effects of the sleep/wake cycle on vulnerability to arrhythmias in animals. The third will be to discuss the potential mechanisms whereby sleep states might conduce to cardiac death in infants.
