Rapid changes in the health care environment are causing organizations to respond to these changes by altering their structures over time. Longitudinal studies, therefore, are appropriate methods for studying the impact of these changes on both the organization and its subunits. However, panel attrition poses a serious threat to both internal and external validity of the findings of these studies (Menaro, 1991; Mohr, 1992). Although specific attention has been paid to the problem of preventing attrition when the individual is the unit of analysis (Given, Keilman, Collins, & Given, 1990; Weinert & Burman, 1996), little or no attention has been devoted to the problems or solutions in organizational studies.The Outcomes Research in Nursing Administration (ORNA) Project is a federally funded longitudinal, nonexperimental causal modeling study that examines the relationship among hospital characteristics (teaching status, size, organizational life cycle, and organizational technology), nursing unit characteristics (skill mix, education, unit life cycle, and unit technology), and nursing unit organizational structure (decentralization, autonomy, nurse-physician collaboration, and the availability and responsiveness of support services) with both patient outcomes (falls, medication errors, and patient satisfaction) and administrative outcomes (nurse satisfaction, team performance, turnover, and cost-efficiency). The study is being conducted at 67 Joint Commission on Accreditation of Health Care Organization accredited general short-term, nonfederal, nonpsychiatric hospitals with greater than 150 beds located in the southeastern United States, Texas, and the District of Columbia. Postinclusion commitment and ongoing communication between the research team and the study coordinators resulted in≥ 70% response rates by staff nurses on all but 11% of nursing units. Only two hospitals and one other unit dropped out of the study postinclusion. Greater than or equal to 80% response rates on patient questionnaires were achieved on all but four units.
