factor for perinatal loss if not recognized; screening programmes that have been recommended overseas have not yet been implemented widely in Australia. Much progress has been made in the management of diabetes. An emphasis on self-education about diabetes has developed over the last 10 years, as has the use of the self-testing of blood glucose levels. The main changes in dietary advice include recommending a higher intake of carbohydrate and fibre and a reduced intake of fat. Hypertension is a major additive risk factor for cardiac and kidney disease in diabetes and should be treated vigorously. Laser therapy has improved dramatically the preservation of vision in those with retinopathy. Knowledge, self-care and motivation of persons with diabetes, their families and the community has been helped by specially interested nurses, dietitians and other nonmedical professionals. Such persons were based initially in major centres but are now to be found more widely. Prevention of both the disease itself and its complications is closer. The importance of diet and exercise
