COLLEAGUES-In a recent analysis of the accessory cell function of AIDS monocytes, Sei et al.[1] reported that although the absolute number of circulating monocytes may be decreased, the capacity of AIDS monocytes to express Ia antigen, a class II HLA molecule critical for T cell activation, is retained. Although we have not observed consistent monocytopenia in patients with AIDS [2], peripheral blood monocytes from patients with opportunistic infections essentially show normal generation of interleukin-1 (table 1) in addition to an intact capacity to act as accessory cells for the optimal proliferation of normal heterologous T cells (table 2). These results are pertinent to the issue of whether there is a monocyte defect in AIDS, because both secretion of interleukin-l and overall accessory cell function appear to be important in achieving effective T cell activation [3, 4]. Taken together with the findings of Sei [1], our data suggest that despite perhaps being infected with the human immunodeficiency virus [5], the AIDS monocyte shows no apparent intrinsic abnormality in the ability to support T cell activity.
