The embryologic explanation generally accepted is that of Warbrickº–founded on the presence of paramedian clefts in the course of lower lip development. Some other defects, which may be related to these same paramedian embryonic grooves, are described by Abramson." Not uncommonly one notices slight indentations, grooves, or dimples in the lower lip paramedian area which may be further minor indications of the embryologic paramedian grooves (Fig. 1, left). There are cleft lip and palate cases with a strong family incidence, but no recognized congenital lower lip sinuses. Some of these patients have a prominent vertical groove in the midline of the lower vermilion, and/or a prominent bulge in each paramedian area. It is suggested that this may be a forme fruste of congenital lower lip sinuses; it can be recognized quite often if one looks for it. It may have significance in genetic counseling.
