During the past three decades, we have witnessed major advances in our understanding of thyroid-system ontogenesis in the fetus and new insights and diagnostic approaches to fetal thyroid dysfunction and management. In addition, important advances in molecular biology and molecular medicine have advanced the nosology and our understanding of the pathogenesis of congenital thyroid disorders. This article reviews current understanding of thyroid system ontogenesis in the fetus as well as the diagnosis and management of fetal thyroid function disorders.
