Pulmonary complications in HIV+ individuals represent a significant cause of morbidity and mortality in AIDS. While pulmonary infections by a variety of opportunistic pathogens are common during AIDS, a number of noninfectious complications of the lungs also are frequently observed during both AIDS and AIDS-related complex. These noninfectious disorders include cardiomyopathy, pericardial and pleural effusions, pulmonary thromboembolism, cor pulmonale, and hypertensive pulmonary vascular disease. I-'Evidence accumulated during the AIDS epidemic has indicated that interstitial pneumonitis (IP) also is a major pulmonary complication of HIV infections in both adults and children.*-" Lymphocytic interstitial pneumonitis (LIP), which is characterized histologically by diffuse infiltration of the alveolar septae and peribronchiolar areas by lymphocytes, plasma cells, and immunoblasts, occurs infrequently in adult AIDS patients (1-296). but is found in pediatric AIDS in more than 50% of the case^.^^.^^ Analysis of the cellular infiltrate in LIP indicates that CD4+ T cells, CD8+ T cells, and B cells can be found in varying proportions in biopsies from patients. lh-I9 Many of these lymphocytes appear to be activated in sitrr, due either to an antigen-specific response in the lungs or to undefined nonspecific stimulation.
