During the past 20 years significant progress has been made in the understanding of the physiology of coronary blood flow in normal and diseased coronary circulation. In the 1960s and 1970s the cardiologist and the cardiac surgeon focused their efforts on the salvage of ischemic myocardium by reducing oxygen demand pharmacologically or increasing supply by coronary bypass surgery. Today, partly as a result of increasingly sophisticated techniques to study coronary circulation, therapeutic interventions are now directed toward the decrease, or even reversal of, myocardial damage caused by coronary occlusion. Our greater understanding of myocardial function and metabolism has been, in part, due to the ability to measure coronary and myocardial blood flow. This paper will review these techniques and attempt to differentiate between the techniques used for measuring coronary artery blood flow and those for measuring regional myocardial perfusion.
