In April 1992, the American Academy of Pediatrics (AAP) recommended a major change in the care of infants in the United States. Their statement recommending supine sleep position for infants was published in the June issue of Pediatrics.1 Many of us who have examined the evidence in detail, however, believe that the risks of such a recommendation may have been underestimated, and the benefits overestimated, for infants in this country. We therefore wish to present an alternative interpretation of the available evidence, so that practicing pediatricians may decide how to advise their patients on the basis of the broadest information, and with the knowledge that the evidence is inconclusive and that the AAP recommendation does not, as the Academy statement clearly acknowledges, serve as a standard of care.
