catheter insertion could be the cause of BCG dissemination, as has been previously reported (5). Adverse side effects, although common, are mild and self-limited. However, more serious life-threatening systemic complications have recently been reported. These include granulomatous hepatitis, pneumonitis, pancytopenia, and BCG sepsis (4,5). Three deaths in the United States have been attributed to BCG instillations (6). Our patient developed biopsy-documented miliary tuberculosis following intraurethral BCG instillations. A 3-day prophylactic course of isoniazid before BCG intravesical administration has been suggested to prevent potential adverse side effects (4). Although this approach needs further evaluation, the systemic complications encountered in our patient and those reported by others indicate the need for effective antituberculosis prophylaxis.
