Beyond the basics: designing a comprehensive response to low health literacy.

Health literacy has gained considerable scholarly attention within recent years as a key factor contributing to disparities in health. In their article,“Health Literacy, Health Inequality and a Just Healthcare System,” Angelo Volandes and Michael Paasche-Orlow (2007) appropriately situate health literacy within an ethical framework. They argue that poor health outcomes resulting from limited health literacy must be construed as injustice on the part of the healthcare system. They are to be commended on pointing out that patients with low health literacy constitute a neglected class of vulnerable patients, given their position as among the least well-off patients in the healthcare system. They propose three avenues by which to redress this injustice: implement universal precautions for limited health literacy, greater use of technology-supported communication, and building clinical incentives. In this commentary, we discuss how their recommendations are laudable, explore the likely difficulties such recommendations would face, and offer additional opportunities to address the problem. We argue that promoting justice requires moving beyond the clinical basics presented in their article in order to comprehensively change the healthcare system.
