A 49-year-old female was evaluated for exertional dyspnea. A transthoracic echocardiogram (TTE) revealed a large mass arising from the proximal aorta, abutting but not compressing the right atrium (Figure 1A). The aortic valve was tricuspid with minimal insufficiency (Figure 1B). Thoracic computed tomography angiography (CTA) showed an 8.1 × 5.2 cm saccular aneurysm arising from the noncoronary sinus of Valsalva (SVA), with normal coronary anatomy (Figure 2). At the time of surgery, the SVA was excised and the defect repaired with a Dacron patch (Figure 3). A postoperative TTE showed normal aortic valve function and a CTA showed complete exclusion of the SVA (Figure 4). The patient had an uncomplicated postoperative course and continues to do well.
