Bleomycin chemotherapy can cause alveolitis, and risk factors include older age (> 70 years) and large dose (> 400 U). Fluorodeoxyglucose positron emission tomography (FDG PET) is being used increasingly to monitor therapy in lymphoma. The authors describe two patients in whom diffuse uptake was seen on FDG-PET images after bleomycin treatment. Because of its ability to detect early metabolic changes, PET showed abnormalities earlier than did other imaging methods, such as chest radiographs. Other causes of alveolitis were excluded. A diffuse pattern of increased FDG uptake in the lungs after chemotherapy may be indicative of bleomycin-induced toxicity.
