Finally, there is the issue of whether to deliver genes locally to discrete areas of disease activity or systemically, where the genes and their products become more generally distributed. Local delivery produces less side eVects and is suitable for autoimmune diseases, such as Grave’s disease, which primarily aVects a discrete anatomical location. Systemic delivery, in contrast, seems better suited to the treatment of disseminated autoimmune diseases like lupus. It is unclear whether local or systemic delivery, or some combination of the two, would be most applicable to RA, which, although primarily aVecting joints, has important extraarticular components.
