A 48-year-old woman with Crohn disease (CD) presented with a 5-day history of bloody diarrhoea, a painful mouth and a pustular rash. Physical examination revealed large, tender pustules affecting the right dorsal foot (Fig. 1a), which broke down to form vegetative plaques (Fig. 1b).
