The hypotensive effect of chlorothiazide has been investigated in 83 patients with hypertension. A significant fall in blood pressure was found in 2/3 of the patients with an average reduction in pressure of 23/15 mm Hg. Plasma volume determinations were made in 10 patients and cardiac output and peripheral resistance was measured in 16. After periods of 7 to 19 days a consistent fall in plasma volume and cardiac output was obtained. The average change in plasma volume was 400 ml and in cardiac index, 0.9 liters/min. After treatment had been maintained for periods exceeding one month no detectable change in plasma volume over the control was found and similarly, the cardiac output was the same as in the pretreatment period. The blood pressure measured directly from the brachial artery remained at a lower level, the average reduction being 39/20 mm Hg below the control period. Therefore there was a fall in peripheral resistance of similar proportions. The average pretreatment value was 1655 and after treatment it was 1265 dynes/sec./cm-5.
