Several groups of experts have considered the issue of planning for cardiac catheterization and cardiac surgical services and have made recommendations concerning staffing and usage [l-3]. Although the suggested minimum levels of use vary somewhat, there is general agreement that optimal quality and cost effectiveness can only be achieved by adherence to some minimal standard for case loads. Studies of the relation between levels of use and both costs and safety have lent further support to this view.An association between low case loads and increased morbidity or mortality has been suggested for coronary arteriography[4, 5] and open-heart surgery [6]. In addition, the cost per examination decreases substantially when volume increases up to about 300 cardiac catheterizations[7] and 100 open-heart operations[7, 8] annually. The national guidelines set forth by the Department of Health and Human Services are consistent with these data and call for a yearly minimum of 300 adult or 150 pediatric cardiac catheterization procedures and 200 adult or 75 pediatric open-heart operations in each cardiac catheterization laboratory or open-heart surgical facility, respectively [9].
