In the cases of pulmonary-valve atresia with intact ventricular septum reported, intramyocardial diverticula and sinusoids ending blindly in the myocardium or communicating with the coronary arteries were demonstrated ante mortem by angiography. Post-mortem confirmation was by serial histologic section of the myocardium and by intracavitary injection of contrast material into the right ventricle, with radiographic visualization of communication and flow of contrast material from the coronary ostia into the aortic root. Since the systolic pressure in the right ventricle must be elevated from early fetal life in these cases it is suggested that the diverticula and sinusoids represent persistence and ectasia of inter-trabecular spaces and their communications with the coronary-artery system.
