The development of nerve stimulation as a method of nerve localization sparked a renaissance in regional anesthesia 30 years ago, as the use of ultrasound has in more recent times. However, despite widespread use of ultrasound and nerve stimulators, nerve injury continues to occur. 1, 2 Both of these technologies allow for only imperfect elucidation of needle position relative to the target nerve, and therefore, neither is reliably able to identify intraneural position of the needle. Despite this, recent research has been able to define better the limitations of nerve stimulation and has produced new concepts, which with further development may enable us to avoid nerve injury. This article will review the new understandings and advances in using nerve stimulation in respect to impedance, threshold current, polarity, and tissue-needle interface.
