Although renal scarring is one of the more severe outcomes of urinary tract infections, it has not generally been one of the factors considered when evaluating uropathogenicEscherichia coli,as it has been assumed that it results from damage caused by repeated infection with the most virulent strains.The authors selected 77 girls, who had had recurrent episodes of acute pyelonephritis, which had been diagnosed by bacteriuria, fever, loin pain and 2 of the 3 criteria: raised C-reactive protein concentration, increased sedimentation rate or reduced renal concentrating capacity. Of these 77 girls 35 had renal scarring defined at urography as calyceal deformity combined with parenchymal reduction. Of 142 isolates available from episodes of acute pyelonephritis the authors only included the 58 strains from infections before scarring. They noted that the patients with renal scarring had a significantly higher frequency of non-Esch. coli.In addition the scarred patients were usually infected with poorly attaching bacteria. The authors hypothesize that renal scarring may be associated with increased host susceptibility and not be related to attaching bacteria.newline˜K.A. Bettelheim
