Progress is inevitable, but its direction is often unpredictable. In the field of genitourinary radiology, few would have predicted that computed tomog-raphy (CT) without contrast material enhancement would become the method of choice for detecting acute stone disease in the urinary tract or that three-dimensional depiction of the renal arteries could be achieved with CT or magnetic resonance(MR) imaging with a single intravenous injection of contrast medium. Like Alice, let us step through the “looking glass” into the unknown.In broad terms, advances in geni-tourinary radiology will likely evolve in several areas: imaging technology, image processing, computer-aided diagno-sis, tissue-specific contrast agents, and “outcomes research.” Current trends that will influence research are the explosion of genetic research, the development of minimally invasive surgical techniques, and the ability to obtain metabolic and other functional infor-mation in addition to traditional morphologic images. Unlike in the past, when advances in technology were considered inherently beneficial, in the future such advances will need to be jus-tified in terms of improved patient outcome.
