Introduction A distinctive cyclical pattern of motility and secretion in the stomach and small intestine of fasting man has been recognized, since the pioneering de-scriptions of Boldyreff, 9 working in the laboratory of Pavlov almost a century ago. Although the phenomenon has continued to interest research gastro-enterologists, a proper appreciation of the underlying mechanisms is only now beginning to be realized and, so far, the practical applications of this knowledge have barely influenced the practice of clinical medi-cine.
