female patients. However, our patient had never used nail polish. Longitudinal mehinonychia results from stimiikition of nail matrix melanocytes, with a subsequent increase in melanin deposition in the nail plate. Pigment deposition may result from greater melanin synthesis by normally nonfunctional matrix melanocytes. or from an increase in the total number of matrix melanocytes which synthesize melanin.''^ In either instance, melanocytes may be normal or abnormal.-"-' It is the unique growth dynamics of the nail plate which result in the linear pattern ol' pigmentation. Mooney and Bennett'' performed a nail matrix biopsy on a patient with periungual hyperpigmentation and longitudinal melanonychia secondary to minocyclinf. and found the presence of a benign lentigo. There is evidence suggesting that long-term use of minocycline increases melanin production by stimulation of melanocytes. causing the muddy-brown pigmentation in sun-exposed areas.A biopsy was not performed in our patient. Indeed, the knowledge that minocycline may induce longitudinal melanonychia may prevent unnecessary biopsy of the nail apparatus. Patients treated with minocycline should be warned of the possibility of cutaneous and nail pigmentary changes. Pigmentation may persist for a prolonged period, despite withdrawal of therapy.
