In December 1994, the Agency for Health Care Policy and Research (AHCPR) released the clinical practice guideline “Acute Low Back Problems in Adults”(1). The AHCPR was created in 1989 as a distinct agency within the Public Health Service charged with enhancing the quality, appropriateness, and effectiveness of health care services and improving access to health care (2). Among its many functions, the AHCPR facilitates the development of guidelines to assist clinicians in the prevention, diagnosis, and management of select conditions (2).Chaired by orthopedic surgeon Stanley Bigos, MD, of the University of Washington, Seattle, the 23-member panel that developed the low back pain guideline included leading clinicians and investigators in surgical, medical, rehabilitative, and diagnostic specialties, as well as generalists and a lay patient. As pointed out by Dr. Liang elsewhere in this issue, a variety of methodologies may be employed in guideline development, ranging in scientific rigor from expert opinion to quantitative synthesis of the relevant literature. The panel relied upon objective, quantitative literature review whenever possible, resorting to expert opinion only when no adequate studies were available.
