PURPOSEAlthough etiology of chronic atrial fibrillation (AF) is unknown, we believe that it is caused by a discontinuity of refractory time of the dilated left atrium in patients with mitral valve disease. Accordingly, we began a modified left-side-only Maze procedure for chronic AF associated with mitral valve disease instead of Maze procedure.MATERIALS AND METHODSWe have operated on 17 patients with chronic AF during the last 18 months. Thirteen had associated mitral valve disease and were treated with this new procedure. Duration of AF history was 1-23 years (mean 8 years). Underlying mitral pathologies were MS and/or MR (3), MSR+ TR (6), MSR+ ASR (2) and MSR+ ASR+ TR (2). Concommitant valvular procedures were MVR (2), MVP by ring (2), MVR+ TAP (5) and MVR+ AVR (2) and MVR+ AVR+ TAP (2). The effect of this procedure and postoperative atrial function were evaluated.RESULTSAF disappeared in eleven cases (85%) postoperatively. Two cases with MSR+ TR returned to sinus rhythm on the third and fifth postoperative day. Postoperative echocardiography showed atrial contraction in the left atrium (73%) and in the right atrium (91%). Recovery of fractional shortening of each atrium showed 0.44+/-0.12 in the right atrium and 0.26+/-0.08 in the left atrium. The size of the left atrium decreased from 54-46 mm in diameter (P< 0.05).CONCLUSIONChronic AF with mitral valve disease was treated with left atrial procedure in over 80% of our cases.
