We have found one-third of siblings of children with overt diabetes mellitus to have an abnormal 2-hr glucose tolerance test by liberal criteria. On retesting and applying more strict criteria, nearly half of these abnormal siblings (26 of 55 retested) had a normal 4-hr glucose tolerance. Their mean insulin responses were significantly greater than those of a more normal group of children. One developed overt diabetes and now receives insulin, 16 have chemical diabetes, and 12 have borderline abnormality.
