Decreased exercise tolerance is a hallmark symptom of patients with heart failure (HF). 1 Earlier studies assessed physical activity levels (PALs) with selfreported questionnaires or a single brief monitoring period by using research grade devices, and the assessment of outcomes typically occurred many years later. 2 As such, little is known about the PAL trends preceding a new diagnosis of HF. By leveraging the AoURP (All of Us Research Program), our pilot study aimed to investigate whether there is potential clinical value to monitor PAL trends from Fitbit devices in high-risk patients with cardiac comorbidities.The study used the Controlled Tier Dataset (version 6) from the AoURP to analyze data from Fitbit devices linked to the electronic health record (EHR) from October 20, 2013 to December 2, 2021. The AoURP and Fitbit data have been described previously. 3 Briefly, Fitbit data consisted of average daily steps and average minutes of graded intensity per day. We analyzed a combined metric of fairly active and very active minutes to reflect moderate to vigorous physical activity.
