EXPERIMENTAL ENDOCARDITIS DUE TO PSEUDOMONAS-AERUGINOSA .2. THERAPY WITH CARBENICILLIN AND GENTAMICIN

Rabbits with left-sided endocarditis due to P. aeruginosa were treated with a high dose (7.5 mg/kg) of gentamicin for 6 days, a low dose (5 mg/kg) of gentamicin for 6 days or 2 wk, carbenicillin (400 mg/kg) for 6 days or 2 wk or a combination of the lower dose of gentamicin and carbenicillin (400 mg/kg) for 6 days or 2 wk. Sterilization of cardiac vegetation was accomplished more effectively in rabbits given the higher dose of gentamicin for 6 days (44% with sterilized vegetation) or combination therapy for 14 days (64%) than in rabbits given the lower dose of gentamicin or carbenicillin alone for 2 wk (23.5% and none, respectively). Combination therapy for 2 wk prevented relapse in all of 6 treated rabbits that were followed after treatment; all 4 animals treated with carbenicillin and 4 of 6 animals treated with the lower dose of gentamicin relapsed after 2 wk of treatment. Levels of creatinine in serum were elevated in 4 of 9 rabbits that received gentamicin alone for 2 wk. Therapeutic success was associated with a synergistic antibiotic combination, a peak bactericidal titer in serum of .gtoreq. 1:16 and a high level of gentamicin in serum.
