To the Editor: Rosenbaum and Jankovic’ reported patienta with focal taak-epecific dieordere. Ten of thew patienta had tremor, and another 10 had tremor and dyetonia. Dysfunction occurred during specific taebe euch ae handwriting, but just with certain typee of writing, and while drinking liquids, but only under certain conditions. Another patient had tongue tremor, but only when the tongue waa protruded or in certain positions. The patients’ complaiita were related to functional dieability occurring during certain tasks. We have also observed many similar patienta we diagnosed ae having easential tremor who had minimal postural or kinetic tremore. These patienta also frequently had a positive family hietory for egeential tremor and often reportad tremor reduction with alcohol ingeetion. One such patient had tremor only while ehaving. Another complained of head tremor while readins becam his head would shake when tilted downward. M e r e complained of ditsculty with drinking liquids but only when the hande were in specific positions. Similarly, tongue tremore would be present only during certain positions. On examination the presence or severity of tremor wan dependent on the precise poeture being maintained or movement being executed. For example, forearm pronation may increase wrist tremor (ehaving tremor) or head tremor may greatly increaee when looking downward (reading tremor).
