Insulin resistance is a characteristic finding in both simple obesity and noninsulin-dependent diabetes (NIDDM).’-’’The presence of insulin resistance in these two syndromes has been documented with the use of a variety of experimental techniques, and has been shown to be rather marked in both. In particular, if plasma glucose is clamped at postabsorptive, euglycemic levels during an exogenous insulin infusion resulting in physiological peripheral hyperinsulinemia (insulin clamp), one finds that whole-body glucose uptake (a measure of the body’s sensitivity ‘to the hormone) is reduced by 40-50% both in obese subjects with normal tolerance to oral glucose and in NIDDM patients with normal body Nevertheless, the former are capable of maintaining glucose homeostasis in response to oral glucose while the latter obviously fail to do so. This simple fact indicates that the same degree of insulin resistance at the whole-body level can have a very different impact on glucose tolerance in different diseases.
