I would like to thank the Thomas William Salmon Committee for this opportunity to look back after more than 30 years of the study of obesity, and to share with you the fruits of these reflections. These reflections come from a special, privileged vantage point-that of the clinical investigator. Part physician, part scientist, the clinical investigator is given the precious opportunity to care for patients in numbers small enough to permit systematic inquiry into their problems. It is a wonderful vantage point, for it reminds us of the goal of our inquiry-the relief of human suffering. It is perhaps appropriate to begin this discussion of obesity with a description of someone in pain and to end it with a 20-year follow-up of this same patient. I The patient, whom I will call Herbert Marx, was 21 years old when I met him 30 years ago, just entering hissenior year of college and thoroughly miserable. A short, awkward young man, he walked slowly into my office, leaning backward a little like a pregnant woman balancing her enlarged abdomen. He had a double chin beneath a small head and a youthful face with smooth pink cheeks and no trace of a beard. He was 5' 5" tall, weighed 205 pounds, and was rapidly gaining weight. The patient said that he had come for help with his obesity which, he said, had gotten completely out of control. Then he explained why he thought that this had happened. "I'm a Jekyll-Hyde character. I'm always going to extremes. There's no middle ground for me. I'm either dieting 100% or gorging 100%. If I break my diet, even a little bit, then I feel that the whole day is ruined. And I feel so terrible that I curse myself mercilessly, but I go right on eating. It's gotten so that this thing dominates my whole life. If I feel slighted, I don't think like other people do, 'Well, it's because I'm Jewish but only that I'm over-
