As an alternative to" defibrination"," consumption coagulopathy" was coined when the complex multiple changes in the coagulation mechanism became more apparent General dissatisfaction with these terms has prompted other suggestions:" intravascular coagulation with fibrinolysis"(ICF)(Bowie & Owen, 1974) and" abnormal proteolytic activity"(APA)(Task Force on DIC, The International Committee for Thrombosis and Haemostasis, personal communication). None of these terms is entirely correct and, in the subsequent discussion, each one, while justified in one particular clinical situation, can be shown to be part of the complex situation that can develop (fig. 1). The abbreviation DIC will be used to describe all forms of this syndrome. What must be stressed is that DIC is not a disease in itself; it is a process with many apparently different aetiologies, and any discussion of diagnosis and management must first examine the underlying mechanisms as they affect man.
