Acute lung injury, more commonly referred to as acute respiratory distress syndrome 35 (ARDS), is a devastating complication of critical illness with a mortality rate of 30-50% 36 worldwide (2). The first cases of ARDS were described over 50 years ago, and in the 37 decades that have followed there have been many important breakthroughs in our 38 understanding of the mechanisms involved. Unfortunately, treatment of ARDS still 39 remains largely supportive; no proven pharmacologic therapies exist, and the discovery 40 of novel therapeutic targets is urgently needed. 41 42Mitochondrial dysfunction is emerging as one such target in ARDS that may be 43 modifiable. Over the past decade, improvements in techniques to image and evaluate 44 mitochondrial function have demonstrated that mitochondrial dysfunction impacts the 45 lung in a multitude of ways beyond ATP production. For example, oxygen sensing, 46 hypoxic vasoconstriction, and surfactant production have all been found to be 47 dependent on a healthy pool of mitochondria (9). These processes are known to be 48 impaired in ARDS. Islam et al. have shown that mitochondrial replacement may be a 49 promising therapeutic approach by demonstrating that stem cells mitigate lung injury via 50 the transfer of healthy mitochondria to the alveolar epithelium (3). 51 52
