The concept that alcohol exerts damaging effects on the gastric mucosa is not new and can be traced back to Beaumont’s [1] obser vations. In animal studies concentrations of ethanol higher than 20% were shown to in duce multiple gastric erosions, the incidence, the number and the severity of which in crease as a function of alcohol concentration [2]. By light and electron microscopy 20-40% ethanol appears to cause progressive disruption of apical cell membrane and cel lular exfoliation with exposure of the lamina propria within a few minutes [3]. Extrusion of mucin and protein into the lumen [3] and histamine release [4, 5] also take place. De spite an evident separation of epithelial cells, intercellular tight junctions seem to remain intact [3].Acute administration of absolute ethanol in rats also results in the labilization of the lysosomal membrane with release of various enzymes, including acid phosphatase [6, 7], which in turn may exacerbate mucosal inju ries [6], Contact between 100% ethanol and the gastric mucosa induces extensive necro sis of mucosal surface and, in a few minutes, severe damage of the mucosal proliferative
