This review article deals with the spectrum of gender identity disorders, the most extreme of which is known as transsexualism. Although described in the early medical literature, these gender dysphoria syndromes have only recently gained acceptance into the formal psychiatric diagnostic nomenclature (the American Psychiatric Association's 1980 and 1987 editions of the Diagnostic and Statistical Manual of Mental Disorders—DSM-III and DSM-III-R). The author briefly describes the early literature on this topic, as a prelude to what he refers to as the modern gender identity movement. The well-known Christine Jorgenson case of 1952 launched the modern era of gender identity research and clinical assessment and treatment of gender identity disorders. These disorders, as defined in DSM-III-R, are described. The following aspects of gender identity disorders are discussed in some detail: body image, clinical evaluation, standards of care, and the trial of cross-gender living.
