PA clinical postgraduate programs are distinguished from on-the-job training in that they have clearly defined learning objectives which are met through a balance of didactic education and supervised clinical experience. The first recognized clinical postgraduate physician assistant (PA) program was the Montefiore Medical Center Postgraduate Surgical Physician Assistant Program, established in 1971. There are now more than 40 such programs in more than a dozen specialties. In 1988, a number of postgraduate PA programs formed the Association of Postgraduate Physician Assistant Programs, which has over the years been instrumental in establishing educational standards for postgraduate PA programs. In 2006 the Accreditation Review Commission on Education for the Physician Assistant adopted accreditation standards for postgraduate PA programs. As PAs move increasingly into specialties and interest in specialty recognition is heightened, there is a great need for research on the relative effectiveness of on-the-job versus formal postgraduate training.
