Aggressive angiomyxoma (AAM) occurs almost exclusively in adult women. A literature survey revealed only 45 cases of this disease in men.The exact pathogenesis is not known. In men, the scrotum, spermatic cord, perineum, pelvic organs, and bladder are predominantly affected. The treatment of choice is complete surgical resection, because relapse is common with incomplete surgical excision.Fifty percent of all recurrences occur within the first 5 years, with 70% of those within the first 3 years.Because of the high recurrence rate, long-term follow-up with imaging techniques is obligatory.Despite its rarity, AAM is a differential diagnosis of unusual and progressive scrotal swelling.
